We are thrilled you have chosen Perfect Dose for your beauty and wellness needs. Booking your first appointment is easy.

Step 1: Simply call us at (480) 745-2496 and a patient care coordinator will reach out to create your patient profile and schedule an appointment.

Step 2: You will receive a text to create your online patient portal.

Step 3: Fill out forms such as medical health history and additional patient forms at their convenience.

Once your patient profile and portal are completed, you will be able to schedule appointments that fit your schedule.
